Patient Portal

RevolutionPHR is our patient portal where you can find eyeglass and contact lens prescriptions, appointments and records for you and/or your children. Send our doctors or staff a HIPAA secure message through the portal and receive documents.

RevolutionPHR utilizes the highest standards of online security while allowing you to view and receive portions of your eye care record through any standard Internet browser on a PC or Mac computer, or mobile device. We encourage you to use the Login information created specifically for you to review the many features available. Contact us by phone or text if you need help.

1. Insert the Username provided to you by our office staff (often your email)
2. Insert the Temporary Password that was created specifically for you, sent separately.
3. This process will lead to a Reset Password screen where you will be prompted to re-enter the Temporary Password and then log your personal Password that must be at least 8 characters long, including at least one number and one capital letter.
