Ask Dr. Nate

qmIn the spirit of making Bright Eyes News and Updates a two-way means of communication, I have created a new category called Ask Dr. Nate. This is your chance to ask any questions you may have about eye disease, vision, infant & child vision, contacts, or other eye-related questions. You can either submit them as comments to this site or them to askDrNate@brighteyestampa.com. I'll do my best to answer in a timely manner.

Please be aware that on this site, I will not be able to make any diagnoses or recommend specific treatment. What I can do is provide general information that pertains to the question and provide links and information that I hope will be useful.

So now I'll just sit back and wait while the questions flood in.... ;^)

Hi. I found your website through the Littlefoureyes.com blog. We live in the Atlanta Metro area and our daughter was diagnosed with congenital esotropia by a pediatric ophthalmologist at two months old. We started patching alternate eyes and at nine months she had her first surgery. The surgery straightened both eyes initially, but 2 weeks after surgery the left eye started turning in again. We got her first pair of glasses and started patching again. Ellie is now 3 and we have seen little to no improvement with the eye turn. The ophthalmologist is recommending a second surgery, but the amblyopic eye is still only at 20/60 for distance vision while the right eye is at 20/35. We have also started doing vision therapy about six months ago. Our ophthalmologist was completely against us trying this avenue, but Ellie's eyes have improved tremendously since we have started. This is a complete out of pocket expense of us, and I want to know your opinion on the matter. We are about to get a second opinion on Ellie before putting her through a second surgery. Do you know of any ophthalmologists that actually stand behind vision therapy? Do you feel that vision therapy can benefit a child that has congenital esotropia/amblyopia/strabismus? Is there any chance for her to ever achieve binocular vision? How long will she have to patch? Would you recommend a second surgery sooner rather than later if the eye turn is severe when the eye is not patched and constant with or without glasses? I asked the ophthalmologist these questions at her last check up, and she said that, no she cannot achieve binocular vision, that she will have to patch until she is 10, that vision therapy has no benefit, and that it is up to me when I want to do a second surgery. I feel like going to ophthalmologist every two months is a complete waste of my time if all I am told is that there is no change and that I should bring her back in two months. I see no benefit from doing that, and I feel like there is more that I should do for her.
On another note, another parent is going through this with her daughter after suffering from strabismus herself as a child and is putting together a forum for doctors and parents on alternative approaches to treating this condition. You can see what she is doing at theeyeofthetigger.com. She wants me to put together such a forum in the Atlanta area and was wondering if you would be interested in speaking for something like that since you are not too far from us. Thanks for your help. I am sorry about my lengthy questions. I appreciate your time! Sincerely, Corrie Koenig

As you know, these complicated situations and there are strong opinions on many sides of the issue. While I can't give you any detailed answers without performing an exam myself, why don't you call the office at 813-792-0637 and we can talk a little bit.
